Job Summary:

 

Under limited supervision, responsible for working within a multidisciplinary team providing clinical care to community adult and adolescent outpatients with serious mental illness, substance use disorder and other co-morbid conditions. *Must have reliable transportation for daily home visits*

Job Duties:

 

  • Evaluates and assesses patient’s behavior, emotional responses, interpersonal relationships, and social conditions.

  • Gives diagnostic impressions and treats adults, adolescents, and families.

  • Develops individual treatment plans for patients and documents case records as needed.

  • Maintains regular contact with each assigned patient to monitor responses to services and identify problem areas.

  • Assists with developing and implementing evidence-based treatment models.

  • Collaborates on additional research and/or contract programs, providing support services and clinical care to community adult outpatients with serious mental illness, substance use disorder and other co-morbid conditions.

  • Responsibilities may include crisis interventions as needed.

  • Performs all other duties as assigned.

    •  

    Education:

     

    Master’s degree in social work, counseling, or psychology is required.

    Experience:

     

    Three(3) years of experience in a health care setting is required.

    1 yr experience with serios mental illness

    Billingual preferred

    Additional Licenses and Certifications:

     

    LPC, LMSW, or LMFTa licensure is preferred.
