Department Summary HSC Health is the academic clinical practice for the Health Science Center. Focused on improving health outcomes to build a healthier community. HSC Health primary and multispecialty care clinics are located on the HSC campus in Fort Worth. Our care teams extend to local hospitals, surgery centers, long-term and continuing care centers, and mobile and in-home care. HSC Health’s interprofessional teams work collaboratively to achieve an optimized treatment plan tailored to the individual patient. Physicians, nurse practitioners, physician assistants, physical therapists, pharmacists, social workers, psychologists, specialized technicians, and nurses work together while intentionally integrating health care students, residents, and fellows to deliver care to our community.
Position Overview HSC Center for Older Adults is seeking to hire a Social Services Coordinator to join their team. The successful candidate should be an MSW. Preferred experience working with older adult populations. Knowledge of community resources for older adult patients is essential. Prior experience working in an interdisciplinary team clinically with emphasis on education of health professionals and experience in clinical research is highly desirable. The successful candidate must possess excellent clinical skills.

Minimum Qualifications
  • Master’s degree in a related field or Licensed Medical Social Worker (LMSW) in the State of Texas.
Knowledge, Skills and Abilities
  • Ability to communicate effectively with a wide cross section of individuals.

  • Ability to think critically

Preferred Qualifications Master’s in Social Work
Required License / Registration / Certification Masters of Social Work

Job Duties

o Work as a member of the interdisciplinary team coordinating the social service needs of older adult patients and their families.
o Responsible for performing direct assessments for older adults in the clinical outpatient setting.
o Provide education and support to caregivers of patients with dementia.
o Identify community resources for older adults and make necessary referrals.
o Follow-up with patients and caregivers to determine and ensure that services provided are helpful, appropriate, and adequate and that patients and families make proper use of services offered.
o Other duties as assigned.
