DescriptionThis is a per diem/on call RN position supporting PACE/ElderPlace North Coast site.Job Description:The Community Care RN provides and supervises professional, independent nursing care for Providence patients. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients’ optimal health and independence.Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more.   • Relevant training applicable to the position, as specified by the Nurse Manager.
  • Upon hire: Oregon Registered Nurse License
  • Within 30 days of hire: National Provider BLS – American Heart Association
  • Upon hire: Relevant certifications applicable to the position, as specified by the Nurse Manager

Preferred Qualifications:

  • Bachelor’s Degree Nursing.
  • 1-year Home health RN experience.
