After the initial onboarding period of up to six months, this position will work a hybrid schedule which includes a combination of working both onsite at Greenwich Drive in La Jolla and remote.

The RN Case Manager for the Care Connection Hub works a hybrid schedule of 4 days at home and 1 day on site. This RN Case Manager calls patients post discharge and uses a standard evidenced-based assessment to make sure patient have what they need and understand their discharge instructions. The RN Case Manager will help coordinate care, schedule appointments, remind patient of red flags to watch for and escalate any issues to provider. Clinical problem solving and Case Management skills are utilized to provide patients with a safe post discharge recovery and connection to outpatient care. The Care Connections RNs work 8 hour shifts, variable days Mon-Sat.

  • Must be a graduate from an accredited school of Nursing.

  • Registered Nurse (RN) license issued by the state of California.

  • Prior case management, utilization review, and discharge planning experience. If insufficient applicant pool, department is willing to consider candidates without case management experience who have three (3+) or more years of recent acute care clinical RN experience.

  • Strong teamwork and interpersonal skills.

  • Must have proven autonomous problem solving, independent thinking, decision making, and prioritizing skills.

  • Proficiency in Information Systems and computer programs including MS Office (Word, Excel, Outlook, PowerPoint, etc.).

  • BSN/MSN

  • CCM

  • Three (3+) or more years of clinical RN experience.

  • Proven home care, managed care, or similar case management experience.

  • Must be able to work various hours and locations based on business needs.
  • Employment is subject to a criminal background check and pre-employment physical.
  • On-site work required during the training period, then hybrid work from home and office when skills are proficient.

If applicable, life-support certifications (BLS, NRP, ACLS, etc.) must include hands-on practice and in-person skills assessment; online-only certification is not acceptable.
