Job Summary
- The Case Manager is accountable for collaborating with the Multidisciplinary Care Team to promote quality patient care. Facilitates efficient utilization of resources.
- Identifies patient care needs throughout the continuum of care.
- In coordination of care through collaborative practice – has excellent communication skills.
- Knowledgeable of the Health Care Continuum and today’s challenge of change.
- Staff is required to maintain a level of physical/mental health needed to perform their duties effectively.
- Adheres to the Nursing Code of Ethics, St. Mary’s Healthcare Center’s Mission, Service Excellence Standards and the standards outlined in the CHI “Ethics at Work” booklet.
Education and/or Experience
- Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.
Licensure/Certification/Registration
- Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure required.
Work Schedule
- Monday – Friday, 8:00am-4:30pm; 80 hours every 2 weeks
- Holidays: on-call rotation
- On-call: every 6th weekend/night
PayThe hourly range for this position is listed below. Actual hourly rate dependent upon experience.
- $29.50 – $44.25 per hour
