Your Opportunity:

The Diabetes Nurse Educator functions as a member of the multidisciplinary team to provide coordination and integration of care, in-depth diabetes education and clinical management to patients and families in the Endocrinology and Metabolism Program. Using an organized approach to problem solving, the Diabetes Nurse Educator will be responsible to assess, consult and mobilize available interdisciplinary and community resources to facilitate outpatient care. Duties may include roles in Diabetes in Pregnancy Clinics, outpatient clinics (Diabetes Centre Calgary), community clinics and inpatient navigation. Responsibilities include assessment, education and ongoing follow up of patients with complex and often decompensated diabetes; provision of education classes; navigation of diabetes services across the continuum of care to inpatients with diabetes and their family members; staff working with inpatients who have diabetes; patients discharged from hospital who do not meet the referral criteria for specialty care; and health care providers from all areas of health care seeking support. The Diabetes Nurse Educator may also be assigned to triage incoming referrals for program clinics and staff, including physicians. The responsibilities and functions of the Diabetes Nurse Educator are performed at full scope of practice. Practices are in accordance with employer policy and professional standards, including Standards of Diabetes Education in Canada and Clinical Practice Guidelines.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour – $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management – Chapters 1 – 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)

Additional Required Qualifications:

This position requires a minimum of 800 hours experience within the last 3 years in direct client diabetes education/management OR current CDECB certification as a Certified Diabetes Educator (CDE). If no current CDE, requirement to obtain within one year of hire. Minimum of 3 years acute care nursing experience required. Minimum of 1 year experience in an outpatient setting working with patients with diabetes and/or diabetes in pregnancy within the last 3 years. Evident knowledge of diabetes medications including antihyperglycemic agents; GLP-1 receptor agonists; DPP-4 inhibitors; alpha glucosidase inhibitors; SGLT 2 inhibitors and insulins required. Evident ability to counsel patients with diabetes on key safety topics including hypoglycemic/hyperglycemic emergencies, to provide diabetes medication/insulin titration advice and incorporate in-depth knowledge of self-management education principles required. Minimum typing speed of 45 wpm required.

Preferred Qualifications:

Baccalaureate Degree in Nursing preferred. Advanced experience with insulin pump therapy preferred. Additional courses and/or certificates in the field of diabetes, change management, client counselling, motivational interviewing, health change approach and client centered care are preferred. Communication and leadership courses are an asset.
