The RN Case Manager is responsible for managing a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community. Job Requirements of the Home Health Registered Nurse Case Manager:

• Valid Nursing License in the State of New Hampshire
• Nursing experience required
• Current CPR Certification required (We offer monthly renewal classes paid for by Interim HealthCare)
• Reliable transportation
