We are community health practitioners, educators, collaborators, and researchers united to reduce socially determined health disparities that are unjust and modifiable. The four main program areas of the FXB Center are: 1) HIV Primary Care, 2) Workforce Development and Quality Improvement, 3) Child Health Nursing, and 4) Research. This position relates to the Workforce Development and Quality Improvement program area.
Posting Summary Rutgers, The State University of New Jersey, is seeking a Program Support Specialist for the François-Xavier Bagnoud Center (FXB Center) in the Rutgers School of Nursing.
The Program Support Specialist (Care Coordination Specialist) will provide HIV case management services to patients enrolled in the FXB Center HIV Clinical Practice programs. The services provided include biopsychosocial assessments, adherence counseling, and referral/linkage to other services to optimize patient health outcomes. This position will also facilitate the continuity of care and provide ongoing support and resources to family members of the patient. Some key responsibilities include:

  • Interview each patient and their family to assess their social, cultural, family financial and continued medical health care needs.
  • Provide adherence counseling to facilitate long-term engagement in care, and to promote better compliance with assigned medication therapies.
  • Track laboratory result for each patient.
  • Provide those living with HIV/AIDS a comprehensive continuum of services through critical linkages with ancillary medical and social support services.
  • Provide referrals to community-based ancillary services, faith-based services, or medical care services as needed.
  • Document and track the linkages of all referrals made.
  • Complete bi-annual social service needs assessment and service plan on each patient or family.
  • Identify and resolve barriers clients may have in following through with their service plan.

COVID-19 Immunization Requirement Under Policy 60.1.35 , Rutgers University requires all prospective employees to provide proof that they are fully vaccinated and have received a booster (where eligible) against COVID-19 prior to commencement of employment, unless the University has granted the individual a medical or religious exemption. Prospective employees who are not eligible for a booster at the time of an offer of employment must provide proof they have received a booster within two weeks (14 calendar days) of eligibility . Based on current guidance, individuals are considered fully vaccinated against COVID-19 two weeks after receiving the final dose of any COVID-19 vaccine authorized or approved for use in the United States by the United States Food and Drug Administration (FDA) and recommended by the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention (CDC). Eligibility for a booster against COVID-19 varies and is explained on the University’s web site located at https://coronavirus.rutgers.edu/covid-19-vaccine . Failure to provide proof of primary vaccination and booster will result in rescission of a prospective employee’s offer of employment and/or disciplinary action up to and including termination.

Qualifications

Minimum Education and Experience
  • Bachelor’s degree in social work or other health profession plus two years of experience in a healthcare setting.
  • Equivalent education, experience and/or training may be substituted for the degree requirement. 
Certifications/Licenses
  • Phlebotomy certification, or ability to conduct phlebotomy services within scope of licensure.
  • BLS CPR certification. 
Required Knowledge, Skills, and Abilities
  •  Proficient in use of electronic medical record systems and Microsoft Office. • Excellent customer service skills. • Ability to manage multiple work priorities. 
Preferred Qualifications
  • Case management experience.
  • Possession of a valid NJ driver’s license.
  • Three years of experience with HIV.
  • Bilingual (English/Spanish).

  • Position requires lifting up to 10 pounds under 1/3 of the time.
