| Organizational Unit Overview | ECU Physicians is the multispecialty group practice of the Brody School of Medicine. ECU Physicians has over 430 providers and sees over 700,000 patient visits per year. ECU Physicians is participating member in the Coastal Plains Network Accountable Care Organization and works closely with Vidant Health to improve the health of the citizens of Eastern North Carolina. This complex organization faces increasing challenges to provide assigned nurses to serve as Health Coaches to assist patients with chronic disease management. The importance of chronic disease management is to not only improve the quality of our patient’s lives but to also assist in meeting required quality measures. ECU Physicians has obtained funding to hire 5 Nurse Specialist Health Coaches in 12-month time-limited positions. The positions will be assigned to primary care clinics but will be centrally managed. The positions will be hired by ECU Physicians but will have a joint reporting relationship with the Director of Nursing for ECU Physicians and HealthAccess, Manager of Case Management. Vidant Medical Group through Health Access has had a robust Health Coach Program and will assist in training and oversight through a management contract. |
| Job Duties | The primary purpose of the Nurse Specialist is to serve as a Health Coach for chronic disease management in ECU Physicians’ primary care clinics. The Nurse Specialist Health Coach would provide 1:1 support, education, oversight of the care provided to patients with chronic diseases. The Nurse Specialist provides coaching and disease management services to identified populations. Facilitates comprehensive care planning and oversees data management to support process improvement. Supports evidence-based practice and medical home/care coordination principles. The Nurse Specialist Health Coach would communicate with all members of the health care team to ensure the patient’s care is coordinated. The Nurse Specialist will serve as role model/educator of all clinical staff in partnership to provide the best care to our patients. The Nurse Specialist performing these duties will be able to bill for services provided thus will need to be knowledgeable of billing practices. In addition, this position will work closely with the Office of Quality and Analytics to assist with quality projects for ECU Physicians such as Meaningful Use, Healthcare Effectiveness Data and Information Set, Merit-Based Incentive Programs, and Physicians Quality Reporting System by closing care gaps. This will be accomplished by the review of records, patient education, and ensuring that referrals for preventive health services are scheduled and documented to close existing and potential care gaps. The position will work closely with the ECU Physicians Medical Director, Chief Operations Executive, Director of Nursing, and Director of Quality and Analytics. The position will report to the Director of Nursing for ECU Policies/employment and to HealthAccess, Manager of Case Management for training and job functions oversight under a management contract for services. The position will have a dotted line to clinic Nurse Manager and/or Department Nurse Administrator of Clinical Operations. CLINICAL SUPPORT POSITION – NOT SUBJECT TO STATE HUMAN RESOURCES ACT. The Brody School of Medicine at East Carolina University is affiliated with ECU Health Medical Center, which serves as it’s teaching hospital, as well as the flagship hospital for ECU Health. |
| Minimum Education/Experience | Graduation from state-accredited school of registered nursing and two years of nursing experience, with at least one year in the assigned specialty area. Must have a current license to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing. All degrees must be received from appropriately accredited institutions. |
