RESPONSIBILITIES:
- Case Finding: Identify eligible cancer patient or their families through multidisciplinary team meeting, Physician referrals, or self-referral. Develop case finding methodologies and pathways.
- Publicity and patient education materials: Develop support program promotional materials.
- Develop and disseminate research materials (surveys, questionnaires, etc); coordinate tasks with hospital’s education and public relations departments and/or IRB.
- Community outreach: develop contacts with community groups, social service agencies, & health care providers.
- Develop patient or family member specific plans of care or support.
- Provide current and competent care: 1) Provide one-on-one counseling and therapy, 2) Lead family therapy meetings, 3) Provide financial counseling and planning where appropriate.
- Health Promotion, Prevention, and Community Advocacy and Education: through providing organized seminars, lectures and published material.
- Provide training and direction to staff interns.
- Coordinate patient care with oncology unit staff.
- Provide competent end of life psychosocial care to cancer patients.
- Coordinate Nurturing Neighbor Program including training and ongoing support to Nurturers
- Provide ongoing supervision and support to Social Work Intern(s)
- Keep abreast of cancer related information relevant to population served.
- Participate in developing promotional materials including articles for newsletter.
