Medical Chart Auditor

Temple University is looking to fill multiple Medical Charts Auditor positions for a 5-year project to better understand the long-term health consequences of a prior SARS-CoV-2 infection.

Essential functions: The EPIC Chart Auditors are responsible for performing thorough chart audits to inform team members of potential inferred, embedded, and missing diagnoses not previously coded; Each auditor will review individual patient records in Epic and document the number of codes that are either missing or incorrectly coded, code text in the notes field, and create codes corresponding with other data elements when not available in the bulk query; Epic chart auditors will additionally share coding corrections with the team so that the working data set can be updated and future bulk data queries can be improved; Discrepancies in coding across chart reviewers will be discussed as a group; Obtains access to and reviews all Epic charts assigned; Reviews all portions of the medical records including but not limited to problem lists, past medical and surgical history, medication lists, specialty and primary care notes, imaging, hospital records, labs and vital signs; In addition to coding text in the notes field and correcting coding errors, the incumbent will keep track of the number of codes that were initially missing and incorrectly coded and the number of codes added by reviewing the notes text; Uses, protects, and discloses patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards; The incumbent will be expected to work independently under general direction of a supervisor; Performs other related duties as assigned.

Required Education & Experience:
Bachelor’s degree in a related field or a current PA license and registration as a professional nurse, and at least four years of relevant experience.  An equivalent combination of education and experience may be considered. 

Required Skills & Abilities:
*Clinical experience in a hospital setting (i.e., medical student, nursing student, resident, physician, nurse, physician’s assistant, etc.) or experience working with medical charts in a medical or research setting.
*Familiarity with MS Office or similar software. 

Preferred:
*Experience with chart abstraction and/or review of medical records for accuracy and completeness.
*Documented proficiency in professional or technical written communication.

This position requires a background check.
This is a grant funded position.
