Lead, Discharge Planning Coordinator, LVN – Social Services – Full Time 8 Hour Days (Non-Exempt) (Union)

The Lead Discharge Planning Coordinator LVN (DPC LVN) provides operational support to the Care Coordination Department to ensure compliance with regulatory requirements for discharge planning coordination and appropriate throughput of patients.  The Lead DPC LVN is accountable to ensure:  1) department orientation is completed for DPC new hires/transfers, 2) department workflows are reviewed, updated and implemented appropriately, 2) DPC documentation standards are updated to reflect current department procedures, and 4) throughput and timely discharges are occurring.  The Lead DPC LVN is responsible for direct supervision of all DPCs including orientation and education of staff.

Minimum Education:
• High School graduate or equivalent.
• Completion of an accredited vocational nursing program.

Minimum Experience/Knowledge:
• Three to five years’ experience as a LVN supervisor
• Ability to work independently with minimal supervision, exercising judgment and initiative.
• Ability to manage multiple tasks with effective prioritization and work effectively in a team environment.
• Typing 40-55 WPM. Experience with computer data entry.
• Proficient in Microsoft Office Suite.
• Strong command of the English language.
• Process oriented.
• Good computer, organizational and customer service skills.

Required License/Certification:
• Valid California Vocational Nursing license required.
• Fire and Safety Certification. If no card upon hire, one must be obtained within 30 days of hire, and maintained by renewal before expiration date. (Required within LA City only)
