Responsible for the planning and coordination of all aspects of  Living Donor Transplant patients’ evaluation throughout the continuation of services to follow up care.  Serves as the primary coordinator for the interdisciplinary team, including physicians, nurses, social workers, dieticians, pharmacists, financial analysts, case managers, and support staff.  Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel and the community.  Collects/documents data to meet clinical, financial, and regulatory requirements.  Participates in the development of guidelines, protocols, pathways and other performance improvement activities.  Participates in research activities.  Serves as a patient advocate throughout the transplant/surgical process.  Represents the Medical Center at regional and national meeting/conferences.

Essential Duties:

  • You will be responsible for the planning and coordination of all aspects of Post Kidney Transplant patients’ evaluation throughout the continuation of services to follow-up care.
  • Serves as the primary coordinator for the interdisciplinary team, including Physicians, Nurses, Social Workers, Dieticians, Pharmacists, Financial Analysts, Case Managers, and support staff.
  • Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel and the community. –

  • Participates in the development of guidelines, protocols, pathways and other performance improvement activities.
  • Participates in research activities.

Educational Requirements:

Graduate of an accredited nursing program required

BSN required

Licenses:

Current, unrestricted California RN License required

BLS from the American Heart Association required

Specialty or Transplant certification required or willing to obtain within one year from start date

Experience:

A minimum of 3 years of Transplant or Acute Care experience required

Demonstrated knowledge of case management required

Experience in data management required

ICU experience preferred

Physical Demands:

Perceptive acuity and physical ability to perform all direct and indirect patients care functions including walking, sitting, standing, bending, and light lifting required.
