THIS IS POSITION IS GRANT FUNDED .

 

JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: Under the direction of Director of Community Initiatives, this position will work in the Center for Youth Advocacy and Well-Being (CYAW) established at UTHSC.  This position will assist youth and families to connect with supportive services through screening and referrals.

 

DUTIES AND RESPONSIBILITIES :

  • Supports and assists children and families with child-serving, educational and human/social services systems. 
  • Collaborates with the grant partners providing technical assistance, training and support to UTHSC and community provider staff, families being served and the community.
  • Engages in community events and provide resources to partners and community members.
  • Manages social media accounts to keep the community advised of events and CYAW programs.
  • Screens youth and families for supportive services and determine options for securing and maintaining resources to support those treatment needs. 
  • Advocates for youth and families with behavioral health, educational, and community partners.
  • Facilitates referrals to appropriate resources and community programs as needed.
  • Provides youth and families with wrap around services determined essential to successful linkage to resources and services. 
  • Attends clinical consultation and supervision as per program requirements.
  • Maintains communication with community agencies and resource providers within his/her assigned region and all confidential information.
  • Generates reports and assist CYAW leadership to fulfill the obligations and outcome tracking of the program and provide follow up to the youth, families, and agencies.
  • Corresponds with potential clients, schools, and officers questions via phone or email. concerning the center and the resources that are provided. 
  • Other duties as assigned.

    Qualifications

     

    MINIMUM REQUIREMENTS:  

     

    EDUCATION: Bachelor’s Degree in Social Services, Criminal Justice, or related field. (TRANSCRIPT REQUIRED)

     

    EXPERIENCE: Two (2) years of related experience.

     

    KNOWLEDGE, SKILLS, AND ABILITIES:

    • Ability to maintain confidential information.
    • Ability to be self-motivated, maintain a friendly demeanor and exhibit tact and professionalism both in person and on the phone.
    • Knowledge and understanding of the system of care values and principles.
    • Demonstrated experience collaborating with diverse stakeholders.
    • Knowledge of Microsoft Office including Microsoft Word and Microsoft Excel.
    • Excellent interpersonal, verbal, and written communications skills.
    • Excellent organizational and time management skills.
