Department Summary HSC Health is the academic clinical practice for the Health Science Center. Focused on improving health outcomes to build a healthier community, HSC Health primary and multispecialty care clinics are located on the HSC campus as well as an off-site obstetrics and genecology location in the Fort Worth medical district. Our care teams extend to local hospitals, surgery centers, long term and continuing care centers, and mobile and in-home care. HSC Health’s interprofessional teams work collaboratively to achieve an optimized treatment plan tailored to the individual patient. Physicians, nurse practitioners, physician assistants, physical therapists, pharmacists, social workers, psychologists, specialized technicians, and nurses work together while intentionally integrating health care students, residents, and fellows to deliver care to our community.
Position Overview The Whole Health Coach is an integrated member of the clinical care team and works collaboratively with diverse patient populations throughout our clinics to engage them in their health and wellbeing. The Health and Wellness Coach collaborates with other clinical team members to create individualized action plans with each patient to support them in achieving their
personal health goals. They are responsible for facilitating the end-to-end process of coaching for both individuals and groups, in-person, and/or over the phone.

Minimum Qualifications
  • Bachelor’s degree in health promotion, public health, exercise science, nutrition, nursing, or other clinical degree and two (2) years related experience; or equivalent combination of education and experience.
  • National Board-Certified Health and Wellness Coach (NBCHWC) within the first 18 months of employment
Knowledge, Skills and Abilities
  • Knowledge of Motivational Interviewing
  • Knowledge of behavior change, adult learning and application
  • Strong clinical and analytical skills in assessing patient health needs, including medical, social and behavioral health
  • Knowledge of chronic diseases, especially hypertension, hypercholesterolemia, diabetes, and pre-metabolic syndrome
  • Knowledge in fitness and nutrition
  • Excellent customer service skills
  • Excellent listening skills
  • Outstanding communication skills in all formats – virtual, phone, electronic, in-person
  • Bilingual skills preferred
  • Ability to be creative and innovative in a fast-paced environment with a lot of ambiguity and constant change
  • Ability to learn and understand medical terminology and organizational departments and divisions to effectively collaborate with clinical colleagues
  • Self-starter; ability to organize and prioritize tasks
  • Ability to work independently and as part of a team
  • Ability to cross-train in other clinic workflows, such as but not limited to answering phones, scheduling, greeting patients and checking in patients for appointments to support team and
  • clinic flow
  • Ability to correctly perform vital signs and screening tools that are appropriate within the Whole Health Coach scope and accurately record data

Preferred Qualifications Master’s degree in Public health, clinical psychology/health psychology, health science, counseling, or social work OR master’s degree in a health related field that is applicable to the clinical environment.
Wellcoaches Lifestyle Medicine Coach/NBHWC approved coaching training program
National Board Certification in Health and Wellness Coaching (NBCHWC)
3+ years of related experience in a health care environment
Required License / Registration / Certification
Work Schedule M-F 8am to 5pm; this is not a remote position
Driving University Vehicle No

Job Duties

o Collaborate with the clinical care team and patient to understand patient health goals.
o Co-create action plans and SMART goals to meet the patient’s stated health goals. Monitor the patient’s progress related to their established goals and provide accountability.
o Provide tailored one-on-one health coaching sessions, utilizing techniques such as motivational interviewing, to patients around their stated health goals, both in-person and telephonically/virtually.
o Supports care team in achieving quality performance outcomes and closing patient care gaps.
o Manage a patient caseload including outreach, scheduling, and patient engagement follow-up.
o Maintain accurate and up-to-date documentation of all patient encounters in EMR. Ensures compliance with all HSC Health/HSC policies and procedures.
o Prepares for, attends, and participates in team meetings and huddles.
