Demonstrates knowledge and skills necessary to provide services based on the physical, psychosocial, educational, safety, and other related criteria appropriate population in his/her assigned area.

Maintains appropriate amounts of forms, materials, and supplies relative to patient census.
Completes assignments without incurring unauthorized overtime.
Does not abuse sick or personal days; maintains personal health and adheres to Absenteeism policy.
Assists with the collection of Medicare letters prior to patient discharge.
Scan documents into medical records as needed.
Call PCP offices to schedule hospital follow up appointments.
Notify Home Health/Hospice Agencies, skilled nursing facilities and PCP clinics if a patient expires or transfers out of the area.
Fax Face to Faces for Home Healthcare/Hospice agencies to patient’s PCP office.
