The Community Health Worker (CHW) is a frontline public health worker who is a trusted and valued team member of the Orthopaedic Trauma Team. The CHW will serve as a link between health care providers, community service providers, and individuals in the community to promote health and address non-medical social needs. The CHW builds individual and community awareness by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, intensive case management, social support and advocacy. The CHW identifies barriers to health and advocates for improving service delivery and other strategies to address non-medical social needs. The CHW will assist patients in accessing appropriate resources, coordinating necessary referrals, providing community and educational support, and serving as a community liaison.

MINIMUM REQUIRED QUALIFICATIONS
High School graduate or equivalent. Community Health Worker certification.
