Job Summary

The incumbent will be assigned to the Citywide Case Management Minna Team providing services to justice involved, formerly homeless, dually diagnosed adults at the Minna, a therapeutic transitional housing site with supports. 

The incumbent will supervise two clinical social workers (CSW I/II) and two social work associates (SWA) who provide specialty behavioral health services to adults involved in the criminal justice system and who have severe mental illness and co-occurring substance abuse.  The incumbent additionally provides treatment for a small caseload.

Department Description

The Division of Citywide Case Management Programs provides comprehensive outpatient treatment to San Franciscans suffering from severe mental illness, often complicated by substance abuse and/or chronic medical conditions. The mission of the division is to help clients recover prior functioning, while reducing reliance on acute services, and facilitate transitions from ZFGH psychiatric inpatient, jails, or other institutions to the community. The Citywide Minna Team uses trauma informed interventions to work with clients who are justice-involved, formerly homeless, and experiencing co-occurring mental health/substance use disorders. Interventions include psychotherapy, crisis intervention, psychotropic medication management, and assertive outreach.

Required Qualifications

  • Master’s degree from an accredited school of social work, possession of a clinical social worker license issued by the California Board of Behavioral Science Examiners, and minimum five years of experience; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position
  • Minimum three years of lead/ supervisory experience
  • Two years’ experience working with individuals actively involved in the criminal justice system
  • One year experience doing community-based case management
  • Two years working with severely mentally ill adults, unhoused, and dual diagnosed populations.
  • Ability to work with clients and program staff from diverse cultural and social class backgrounds
  • Ability to ensure patient care service standards are consistently met; assists staff with managing challenging patient interactions with sensitivity and skill.
  • Knowledge of psychiatric service operations and ability to apply concepts in an acute care setting.
  • Strong interpersonal skills and experience working with populations that is diverse in gender, sexual orientation, and ethnicity.
  • Ability to work cooperatively and communicate effectively with others.  Ability to work in a high stress, high volume area and work well under pressure and with deadlines.  Ability to follow oral and written instructions accurately; detail oriented.  Able to efficiently prepare written summaries of activities, set priorities and complete reports within required time frames.  Knowledge of medical and/or psychiatric terminology.

Preferred Qualifications

  • Knowledge of medical center, HR and UC policies and processes. Knowledge of relevant psychiatric health services internal policies and external regulations.

License/Certification

  • Licensed by the California Board of Behavioral Sciences as a LCSW, MFT, or LPCC
  • Completion of California BBS CE requirements to provide clinical supervision to AMFTs, ASWs,and APCCs
